Huh. What I do: I make a shape. and then spam the shape all over the map. then make lines, via 3,2,7, and 6 tiles through everything. Then I mess around, and make things work.

I base my map around mechanisms or a particular mechanic think through the most complicated way in which it can all fit. I then build these mechanisms test them, modify them and radically change the layout of my map, test again, go get some coffee, test again and finally I start to think about tiles after that.

@Inspired that is quite a good method but there is a drastic change between steps 2 and 3 maybe you should go into more detail about precicely how you acheive that.

In this map, I will make the tileset to top of the map, and the focused enemy is on rockets.
First, I basically make a -not random- shape from E tiles.

Then, I place my ninja and some enemies.

I make tiles better.

I add gold to map and fix parts that is not good.

Adding mines and some more stuff that I see it's missing.

More changes and playtest.

Done!
How I make the tiles and the map are explained in this post. Well, I use Inspired's technique.

I usually just think about some cool picture, and then start drawing it with the shapes of the tiles on paper. When that is done, I open my computer and start making the map.
